Lexiscan(Regadenoson) – replaces Adenosine in stress testing
Doripenem
Collengenase (Santyl) – replacement for Papain products from FDA action

Deletions:
Iberet folic 500 (by manufacturer)
Urised (by manufacturer)
Colchicine injection (by FDA)
Edrophonium (by manufacturer)
Esmolon injection (by manufacturer – premixed infusion still available)

Heparin per protocol

Heparin Protocol Orders orders are now available online.  When writing heparin per protocol, please specifiy thrombolytics yes/no, and whether CVA, AF, DVT/PTE, STEMI or Non-STEMI is involved. The check boxes for these are on the order form.
