The April 8th Wall Street Journal contained an interesting editorial on the utility of “quality metrics” in health care entitled ‘Why Quality Care is Dangerous’.
The Obama administration is working with Congress to mandate that all Medicare payments be tied to “quality metrics.” But an analysis of this drive for better health care reveals a fundamental flaw in how quality is defined and metrics applied. In too many cases, the quality measures have been hastily adopted, only to be proven wrong and even potentially dangerous to patients.

Computerized clinical documentation is coming in about 12-18 months. An early preview is located on BaptistMD. Under the ‘help’ dropdown menu is a pdf file labeled “Clinical Documentation CD”.
On the hospital computers, you may open the web browser and enter “baptistmdtrain” as the URL and then enter “portal” as the patient last name. This will bring up an example patient file and their clinical data.

If you print a blank page, click on print again, then click on the advanced tab and make sure “print as an image” is checked.

A new admission progress note form has been adopted which incorporates documentation requirements regarding “Present on Admission” (POA) diagnoses to distinguish these from “Hospital Acquired Conditions” (HAC’s). See the sidebar item HAC’s and POA’s for more information.

The ED has begun the use of new software to print out discharge instructions, work or school excuses, and prescriptions for our patients. The program is called ExitCare and an online demo of ExitCare is available.
ExitCare has been well accepted by the physicians and nurses in the ED, and has added to our productivity while increasing information we can provide to our patients.

Baptist Memorial Hospital Golden Triangle physicians are expected to utilize this resource for any order set they may need in the hospital, in their office, or even at home. Printing out these orders and then faxing them in to the appropriate nursing unit with patient identifying information on the fax is considered acceptable use. Hospital Fax Numbers
